Prior to your first appointment, please print the Registration form and bring the completed form with you to your first appointment.   

You may use the Records Release form to request that your records be sent to us from another provider's office. 

If your health history is complex, or if you have a specific goal in mind, please complete the Health History form prior to your first appointment.
